In connection with Regulation (EU) 2016/679 of the European Parliament and of the Council of April 27, 2016 on the protection of natural persons with regard to the processing of personal data and on the free movement of such data, and repealing Directive 95/46/EC, we hereby inform you that from May 25, 2018, you will have the rights specified below in relation to the processing of your personal data by Biomedika Sp. z o.o., ul. At the same time, pursuant to Article 13 of the Regulation, we would like to inform you that from May 25, 2018, the following information and rules related to the processing of your personal data by Biomedika Sp. z o.o., ul. Księcia Witolda 15/10, 50-202 Wrocław, will be valid.

3. The facility processes your address data for the following purposes:

3.1. communication regarding planned visits

3.2. the possibility of canceling or confirming a visit

3.3. reminders about visits

3.4. scheduling another visit

3.6 informing you about changes in the product offer in order to present a better price offer pursuant to Article 6(1)(a).

3.7 promotional activities of the company in order to present new health-promoting solutions pursuant to Article 6(1)(a).

3.8 marketing activities by sending text messages pursuant to Article 6(1)(a).

4. Your personal data will not be disclosed to other recipients or categories of recipients of personal data.

5. Your address data will be processed for the period necessary to achieve the processing purposes indicated in point 3:

a) in the scope of communication, arranging and canceling visits for the period of providing information by the owner of personal data about the deletion of personal data or liquidation of the facility

b) in the scope of the facility’s marketing activities and informing about new products and changes in the offer of visits for the period of providing information by the owner of personal data about the deletion of personal data or liquidation of the facility

c) data related to sending text messages will be deleted after canceling the text message subscription or deleting personal data from the database by their owner

6. In connection with the processing of your personal data by Biomedika Sp. z o.o., you have:

a) the right to access your data, pursuant to Article 15 of the Regulation,

b) the right to rectify your data, pursuant to Article 16 of the Regulation,

c) the right to erase your data, pursuant to Article 17 of the Regulation,

d) the right to restrict data processing, pursuant to Article 18 of the Regulation,

e) the right to object to data processing, pursuant to Article 21 of the Regulation,

f) the right to data portability, pursuant to Article 20 of the Regulation,

7. In cases where the processing of your data is based on Article 6(1) of the Regulation (i.e., consent to the processing of personal data), you have the right to withdraw this consent at any time, without affecting the lawfulness of processing based on consent before its withdrawal.

8. If you believe that the processing of your personal data violates the provisions of the General Data Protection Regulation of April 27, 2016, you have the right to lodge a complaint with the Inspector General for Personal Data Protection.

9. Providing your personal data is voluntary, but necessary to provide the services offered by Biomedika Sp. z o.o. and to maintain customer contact.

10. Biomedika Sp. z o.o. collects the necessary amount of data from you to provide the services consistent with the offer you selected.

11. The customer receiving SMS notifications:

a) consents to receiving commercial information electronically to the provided email address and/or telephone number within the meaning of Article 10, Section 1 of the Act of July 18, 2002. on the provision of electronic services by the Seller

b) consents to the processing of data for marketing purposes by the Seller

c) consents to the use of e-mail and/or text messages for marketing purposes by the Seller in accordance with the Act of 16 July 2004 – Telecommunications Law
