§ 1
1. The gift voucher is issued either as an elegant invitation printed on eco-friendly paper and packaged in an eco envelope, or as an electronic voucher.
2. The condition for issuing the voucher is full prepayment of the service offered by the Biomedika Center for Medicine and Trichology to the account:
96 1090 2398 0000 0001 4833 5783 Santander Bank Polska, and acceptance of these Terms and Conditions.
3. Gift vouchers are available for purchase at any time and may be issued for any chosen amount.
4. The validity period of the gift voucher is 12 months from the date of purchase. The expiry date cannot be extended.
5. At the moment of purchasing a service from the Biomedika Center for Medicine and Trichology, the Client declares whether they wish to receive a voucher.
6. If the gift voucher is not used within its validity period, the Client loses the right to use the service covered by the voucher.
7. The Client has no right to exchange the gift voucher for cash.
§ 2
1. To use the voucher, the Client must book an appointment for the service specified in the voucher. The voucher may be used only once.
2. When booking an appointment with a voucher, please inform us that the payment method will be the voucher.
3. If the booking is not cancelled at least 24 hours before the scheduled visit, the voucher will be considered redeemed.
4. When redeeming the voucher, the Client must always bring the voucher to the appointment.
5. The voucher may be used in parts. No cash refund is given if the voucher exceeds the value of the service. If the service value exceeds the value of the voucher, the Client must pay the difference.
6. Payment with a gift voucher cannot be combined with promotions or discounts.
7. Personalized (name-specific) gift vouchers are non-transferable.
8. Gift vouchers issued for specific services cannot be exchanged for other services offered by the Biomedika Center for Medicine and Trichology.
9. Value-based gift vouchers entitle the holder to use any treatment of choice.
10. The Biomedika Gift Voucher entitles the holder to use any treatments except scalp and hair treatments.
11. TrichoSpa Gift Vouchers apply exclusively to scalp and hair treatments performed within the Biomedika Center for Medicine sub-brand – TrichoSpa.
§ 3
1. Vouchers are sold directly at the Biomedika Center for Medicine and Trichology. They may also be purchased by placing an order through the contact form available at www.biomedika.com.pl or www.trichospa.pl, via email at biomedika.trichospa@gmail.com, in the online store, or by phone.
2. Payment for vouchers purchased directly at the Biomedika Center for Medicine and Trichology may be made in cash, by card, or via BLIK.
3. For vouchers ordered online, payment must be made by transfer to the account: 96 1090 2398 0000 0001 4833 5783 Santander Bank Polska. Order confirmation and payment details will be sent by email.
4. For bank transfers, treatment bookings may only be made after payment has been credited to our account.
5. Delivery costs of the gift voucher are borne by the Buyer.
§ 4
1. Any complaints regarding the improper performance of obligations by the Biomedika Center for Medicine and Trichology under these Terms and Conditions must be submitted directly at the Center within 3 days of performance of the obligations.
2. The Client will be informed of the outcome of the complaint by email.
§ 5
1. The Data Controller of the Client’s Personal Data is Biomedika Sp. z o.o., ul. T. Kościuszki 29 (Building A, 1st floor), 50-011 Wrocław, biomedika.trichospa@gmail.com.
2. Personal Data are processed with appropriate security measures in compliance with legal requirements.
3. Personal Data will be processed for the following purposes:
a) execution of the voucher,
b) settlement and invoicing of services,
c) handling of complaints (if applicable),
d) marketing purposes, where the legal basis for processing is the legitimate interest of the data controller – Art. 6(1)(f) GDPR (Regulation (EU) 2016/679).
4. In addition, Personal Data may be used for statistical purposes within the legally justified interest of the data controller (Art. 6(1)(f) GDPR).
5. Providing Personal Data is voluntary but necessary for voucher execution.
6. Personal Data will be stored only for the period necessary to achieve the purposes listed above and, after their fulfillment, for the period required by law, including (i) civil liability regulations regarding the proper performance of obligations, and (ii) tax regulations.
7. The Client has the right to: access their data, request rectification, erasure, restriction of processing, data portability, object to processing on grounds relating to their particular situation or for direct marketing purposes, and withdraw consent at any time without affecting the lawfulness of processing carried out before withdrawal.
8. If the Client believes that the rules on personal data protection have been violated, they have the right to lodge a complaint with a supervisory authority, in particular in their country of habitual residence, place of work, or place of the alleged violation.
9. The legal basis for data processing is Art. 6(1)(a), (b), (c), and (f) GDPR.
§ 6
1. These Terms and Conditions may be amended at any time, provided that vouchers purchased before the change remain subject to the previous provisions.
